Who we are:
Housed in San Francisco and backed by large Series A funding, we are a passionate, hardworking team dedicated to cutting-edge healthcare technology solutions

What we do:
We are developing the next generation platform for care coordination of patients with complex chronic diseases. Our solution will enable provider institutions to successfully manage the clinical and behavioral issues that drive poor outcomes and high costs in these patients. Moreover, the platform will allow care coordinators to successfully achieve these goals while minimizing disruptions to physician workflows.

Our current development partnerships include:
- Medical centers who are providing real-time access to their EHR data
- A data analytics company that is providing structured analytics and natural language processing to create our analytics platform
- Large medical institutions who have signed up for pilots
- Researchers at premier universities including MIT and Columbia
Responsibilities
Provide clinical leadership with the design, implementation, and adoption of Acupera’s advanced care coordination platform
Lead organizational efforts in identifying and using clinical and behavioral data to support care coordination intelligence system
Drive the creation of disease risk models for Acupera’s system
Engage care providers in the development and ongoing use of Acupera’s system
Establish and lead processes for integration with EHRs and other health information systems to enable effective care coordination
Manage strategic partnerships with MIT, Columbia, and other key institutions
Build relationships with potential customers to gain support for key IT initiatives that enable new care models
Skills
Robust knowledge of clinical information systems, clinical documentation, and data management
Strong analytical skills and understanding of data-based approaches to problem solving
Expertise in EHRs, data integration and mapping, and data mining
Familiarity with SNOMED and other medical dictionaries
Strong interpersonal, written, and oral communication skills
Keen understanding of data elements and where they exist in health system databases
Education
MD plus residency from a top university
Health informatics education with experience in its application to clinical setting
